Consent

By signing up and using the Cetasika Life services through the Cetasika Website, the Cetasika Application, or the Cetasika Clinic Management System (each as defined in the Cetasika Life Privacy Policy, and collectively referred to as the “Platform”), I hereby express consent and agree that:

1. Application Role Acknowledgement

(the term “Platform” is used throughout this Consent to refer to the Cetasika Website, Cetasika Application and Cetasika Clinic Management System, as defined in the Cetasika Life Privacy Policy)

  • I understand that Cetasika Life Private Limited is a technology Platform and facilitator only.
  • Cetasika Life does not provide medical, wellness, diagnostic, or fitness services.

2. Medical & Wellness Disclaimer

  • Any consultation, advice, or service is provided solely by independent Service Providers.
  • Information on the Platform is not a substitute for professional medical advice, diagnosis, or treatment.
  • I agree to consult qualified professionals before making health decisions.

3. Data Collection & Sharing Consent

I give my consent to the collection, storage, processing, and sharing of my personal and health data:

  • In accordance with the Digital Personal Data Protection Act, 2023
  • Solely for service delivery, legal compliance, and platform operations

I understand that my data may be shared with Service Providers involved in my booking.

4. Accuracy of Information

  • I confirm that all the information provided by me is relevant, accurate, updated, and complete.
  • I accept responsibility for the consequences arising from incorrect or incomplete information.

5. Appointment & Payment Consent

  • I understand appointments are subject to Service Provider availability and confirmation.
  • Payments and refunds are governed by Cetasika Life Private Limited policies.
  • Cetasika Life is not responsible for cancellations, delays, or non-performance by service providers.

6. Telemedicine Consent (Where Applicable)

  • I give my consent for teleconsultations as per Telemedicine Practice Guidelines, 2020.
  • I understand the limitations of remote consultations.

7. Liability Waiver

I agree that Cetasika Life shall not be liable for outcomes, injuries, losses, or damages arising from services provided by Service Providers.

8. Acceptance of Legal Documents

  • Refund & Cancellation Policy
